If you access a QR-activated program page, your name, acknowledgments, and submission timestamp are recorded as part of the digital waiver process.

We use the information we collect to:

• Fulfill and document digital waiver agreements

QR-activated pages require completion of a digital waiver prior to access. By completing the waiver, you agree to the terms stated therein. Access is intended solely for members of clubs with a current license agreement with PRIMED. Performance LLC.

The Site may link to third-party websites. PRIMED. assumes no responsibility for their content or practices and inclusion of any link does not imply endorsement.

These Terms are governed by the laws of the State of Florida. Disputes shall be resolved exclusively in the state or federal courts of Broward County, Florida.

PRIMED. may modify these Terms at any time. Continued use of the Site after changes constitutes acceptance.

These Terms and the Privacy Policy constitute the entire agreement between you and PRIMED. Performance LLC regarding your use of the Site.
